1^st time patients with referral form/new patient referral form due to the old one is expired

• Original Patient Referral Form
• ID Card (Original) or other official documents

Patients who have been applied insurance service and need to take the continuing treatment/old referral form has not been expired, patients according to an act; section 7 (Gold card privilege/handicapped/veteran), and patients according to an act; section 8 (vacancy rights)
